is witnessing a ‘health transition with infectious
diseases declining due to immunization and control programmes
and chronic non-communicable disease (NCD) increasing due
to increasing life expectancy, life style changes and urbanization.
Changes in how medical care for chronic care diseases is
financed, managed and delivered are necessary to prevent
development of acute episodes of illness and this is the
space where the Chronic Care Foundation endeavours to step
in and make a sustainable difference.

To
promote good health by proactively minimizing the incidence
and effects of chronic disease in society.
Mission
CCF
is committed to empowering the community with an accessible,
efficient health care system that improves the safety and
quality of care of patients with chronic, cardiovascular
disease, diabetes and kidney disease in India; through prevention,
advocacy, education and collaboration amongst stakeholders
within the community.
Objectives
The
Chronic Care Foundation has been founded to:
- Generate,
collect and serve as a one point source of information
on population health statistics related to Cardiovascular
Disease, Diabetes and Kidney Disease.

Identify options for early detection, prevention and management
of the identified chronic disease and develop replicable
intervention strategies, guidelines and best practice
protocols for diagnosis, management, counselling and financial/social
support.

Advocate health promotion and chronic disease prevention
and control in the region. Promote health, especially
for poor and disadvantaged populations. Slow and reverse
the adverse trends in the risk factors for non-communicable
chronic diseases.

- Provide
a platform for medical practitioners and other stakeholders
to learn best practices in patient care, and gain and
share knowledge by stimulating training and capacity building.

Provide a platform and develop guidance packages for patients
to gain information about their condition, discuss and
participate in management decisions and catalyse patient/family
awareness of disease, therapies and consequences.

Raise awareness of chronic non-communicable diseases and
ensure that the foundation Influences public healthcare
policies.

Link regional networks and international partners in collaboration
for chronic noncommunicable disease prevention control
and management.

Provide accessible long-term supportive welfare services
for continuous care, and facilitate with any other services
that maybe required in connection with the above.
